San Francisco 49ers starting wide receiver Torrey Smith fell to the ground while attempting to haul in a 25-yard throw by Colin Kaepernick against the New York Jets. Smith remained on the field and was immediately looked at by training and medical staff.

Smith landed on his head and was possibly knocked out. He sat up and was seen talking with team staff. Smith left the field on a cart, which took him to the 49ers' locker room for evaluation.

The 49ers announced that Smith had suffered a concussion. He will not return to the game.

Statistically, this is Smith's worst season of his career. He has 267 yards and three touchdowns off of 20 receptions. Smith signed with the 49ers as a free agent in 2015.
