Witherspoon left Sunday's game against the Seattle Seahawks with a knee sprain in the first quarter.
"He'll have an MRI tomorrow (Monday)," head coach Kyle Shanahan told reports after the game.
Witherspoon attempted to tackle running back Chris Carson and went down out of bounds. Trainers eventually helped him to the locker room. The second-year corner did not return to the game.
The 49ers made Witherspoon a third-round draft pick out of Colorado in 2017. He has started 21 games over his two seasons and registered 69 tackles, 11 passes defensed, two interceptions, and a forced fumble.
Witherspoon struggled early on this season but has rebounded well toward the end.
"He's a good player," veteran cornerback Richard Sherman said last week of Witherspoon. "I think a lot was made over a few plays that didn't go his way, a few mistakes. But I think overall, if you take the whole season, he's had a pretty good year."
