T Jaylon Moore (quad, questionable)


Jaylon Moore stayed down on the field after a play in the third quarter but eventually managed to walk slowly to the sideline before heading to the medical tent for evaluation. The 49ers later announced that Moore is questionable to return due to a quad injury.


Guard Spencer Burford stepped in to replace Moore at left tackle.

G Aaron Banks (knee, out)


Aaron Banks was assisted off the field after sustaining an injury on a play in the fourth quarter. The 49ers quickly ruled him out with a knee injury. Nick Zakelj replaced Banks at left guard.
