Alex Smith leaves the game with an injury

While trailing the St. Louis Rams 14-0, San Francisco 49ers quarterback Alex Smith suffered a concussion. After the play that injured him, he managed to lead the 49ers to a touchdown drive and get them within seven points of the Rams. He was pulled out of the game after that drive and the 49ers say he will not return.

Colin Kaepernick has replaced Alex Smith.

The 49ers trail the Rams 14-7.
