It's a combination of things. Outside of the DL, their defensive personnel is garbage, and Jack Del Rio is a lousy head coach. He's a "defensive-minded" coach, yet their defense has sucked every year he's been there.

He never should have been a HC we should have paid him big $$ to stay as a DL coach.
I'd bring Norton in here as a LB coach in a heartbeat

But they don't have a DPOY caliber player. Mack is - and always has been - inconsistent. He puts up one massive statistical game and then disappears for 3 weeks. That's exactly what happened when he won the award. He's overrated as hell.

It is a complete s**tshow over there. Players sending subliminal shots at JDR on Twitter and declining interview requests, Khalil Mack and Bruce Irvin sitting out practice for "non-injury related" reasons, etc.They've inherited our dysfunction.
